HOME BASED SERVICES
Trusting Hands provides home based services to individuals who live in and around the Sullivan County area. Home based services allow individuals to maintain the highest level of independence possible while continuing to live at home. These include:
Short and long term care- This service provides a variety of care options that address activities of daily living. These services options might include:
Individual and community based health education- Individuals and employers may access a host of educational resources and in-service trainings related to health care. A partial listing of topics/services covered by this program include:
Rehabilitation services- Services are available through the Alliance that promote improvement in mobility, dexterity and communication that may have occurred as a result of a health crisis episode. Treatment may work towards improved:
Nursing Consultation Services- A full component of skilled nursing services are available for traditional home health treatment. Services are provided by certified or registered nurses consistent with a treatment plan established by the patient's attending physician and are available on a 24-hour basis.
