Trusting Hands Resources for Independent Living (THRIL)
Community Alliance has, through its fully licensed home health care agency, Trusting Hands Home Health Care, implemented a program to promote the health, safety, and increased capabilities of seniors living in the Newport, Claremont, and Charlestown communities. The program is designed to assist participants in maintaining their quality of life and independent living status.
The program’s goal is to identify risk factors that may impact an individual’s ability to live independently, and to identify programs and supports that would help decrease those risk factors.
THRIL offers the following services:
Private, in-home risk assessments conducted by a licensed nurse,
assessing the types of supports and services currently being utilized by
the
participant, as well as identifying those untapped resources which would assist
seniors in maintaining their independence. On-going follow up with the nurse is
available to ensure goals are met.
Monthly wellness programs covering topics of interest to seniors that promote healthy safe lifestyles and increase opportunities for socialization.
Free monthly blood pressure clinics
A health and independent living telephone hotline, available during normal business hours, to seniors, their family members or caretakers, as well as housing managers, to answer (non-emergent) questions related to health and/or the aging process.
An area resource guide of services and supports available to area elderly with an individualized resource section utilized as part of the assessment.
